Gastroscopy

Comprehensive care for digestive disorders by Dr. Akeel Alisa

What is a Gastroscopy?

A gastroscopy, also known as an upper endoscopy or oesophago-gastro-duodenoscopy (OGD), is a procedure that allows a gastroenterologist to examine the upper part of the digestive system. Using a thin, flexible tube with a camera and light (endoscope), the doctor can view the oesophagus (food pipe), stomach, and duodenum (first part of the small intestine) in detail.

Gastroscopy is commonly used to investigate symptoms such as persistent heartburn, difficulty swallowing, nausea, vomiting, abdominal pain, indigestion, anaemia, or unexplained weight loss. It is also an important tool for detecting ulcers, inflammation, polyps, and early cancers.

Why is Gastroscopy Important?

  • Accurate diagnosis of upper digestive symptoms – Gastroscopy provides direct visualisation of the oesophagus, stomach, and duodenum, giving clearer results than X-rays or scans.
  • Early detection of cancer and precancerous changes – It allows doctors to detect conditions such as Barrett’s oesophagus or stomach cancer at an early stage, when treatment is most effective.
  • Biopsies can be taken – Small samples of tissue can be collected during the procedure to test for conditions such as Helicobacter pylori infection, coeliac disease, or cancer.
  • Treatment during the same procedure – Gastroscopy can be used to stop bleeding, stretch narrowed areas (dilatation), remove polyps, or insert feeding tubes.

Who Should Have a Gastroscopy?

Your doctor may recommend gastroscopy if you experience:

  • Ongoing or severe heartburn and acid reflux
  • Difficulty or pain when swallowing (dysphagia)
  • Unexplained nausea or vomiting
  • Black stools (possible bleeding in the stomach)
  • Anaemia (low iron levels) without a clear cause
  • Unexplained weight loss or loss of appetite
  • Persistent indigestion or bloating
  • Suspected stomach or oesophageal cancer

Preparing for Gastroscopy

To ensure a clear view, your stomach must be empty:

  • You should not eat or drink for around 6 hours before the test.
  • Your doctor will give you specific instructions depending on your health and medications.

What Happens During the Procedure?

  • Gastroscopy is usually performed as an outpatient procedure.
  • You may be offered a local anaesthetic throat spray and/or sedation to make you comfortable.
  • The endoscope is gently passed through the mouth, down the oesophagus, and into the stomach and duodenum.
  • The doctor examines the lining carefully, takes biopsies if needed, and may carry out treatment such as stopping bleeding or dilating a narrowing.
  • The procedure usually takes 10–20 minutes.

You will be monitored afterwards until any sedation wears off and can normally return home the same day.

Risks and Safety

Gastroscopy is a very safe test. Complications are rare but may include:

  • A sore throat for a few hours afterwards
  • Minor bleeding after biopsies or treatments
  • Very rarely, perforation (a small tear in the lining)

Your specialist will explain the risks and answer any questions before the test.

Benefits of Gastroscopy

  • Clear and accurate diagnosis of upper digestive symptoms
  • Ability to detect and treat problems in one procedure
  • Early identification of cancers or precancerous changes
